• Contact Information: Full name, email address, phone number, and residential address.
• Health History & Intake Data: Medical conditions, medications, surgeries, injuries, treatment goals, and clinical assessment notes recorded by your Registered Massage Therapist (RMT).
• Insurance & Direct Billing Details: Extended healthcare policy numbers, member/certificate IDs, and carrier names for real-time electronic claims submission (e.g., Pacific Blue Cross, Telus Health, Sun Life, Canada Life, Green Shield Canada).
• Appointment History & Financial Records: Dates of treatment, duration, fee schedules, and payment receipts.

3. Purpose & Use of Your Information

Your information is collected and used strictly for legitimate healthcare and administrative purposes, including:

• Conducting accurate orthopedic assessments and formulating personalized treatment plans.
• Ensuring safe manual therapy by identifying contraindications or precautions.
• Processing direct billing claims to your extended health insurer upon your authorization.
• Sending automated appointment confirmations, Jane App schedule reminders, and digital receipts for tax and personal records.

4. Secure Storage & Jane App Clinical Infrastructure

All electronic medical records, clinical charting, and intake documentation are managed through Jane App (Jane Software Inc.), a Canadian healthcare practice management platform.

Jane App adheres to the highest global and Canadian security standards, including:

• Canadian Data Residency: All patient data is stored on secure servers located within Canada.
• End-to-End Encryption: 256-bit SSL/TLS encryption for all data in transit and AES-256 encryption at rest.
• SOC 2 Type II & HIPAA / PIPEDA / PIPA Compliance: Rigorous third-party security audits and privacy controls.
• Role-Based Access: Only your licensed practitioner has access to your confidential clinical health records.

5. Disclosure of Information

We do not sell, rent, or trade your personal health information under any circumstances. Your information is only disclosed with your explicit written consent, or as required by law (such as a formal court subpoena or statutory public health requirement).

6. Accessing & Correcting Your Records

You have the legal right to access your personal health records and request corrections if you believe any information is inaccurate. To request a copy of your records or update your contact details, you can log in to your secure Jane App patient account or contact our privacy officer.
